Above is the allergy notice I created for someone who had an allergy to macadamia nuts and their oil. I thought that by customizing it for him/her, the card would be more simple and easier to read. Copy and paste the sentences in Microsoft Word and print it on colorful paper, and you will be able to create your own bilingual allergy notice.
Below is the template. Type your allergens in the ( ) in the middle and create yours.
